2 free regular cleanings per year, including checkup exams, free take-home whitening kits all with no additional fee.*
X-Rays: 1 set of regular bitewing X-rays and any other X-rays necessary.
30% OFF most services including fillings, crowns, bridges, dentures, partials, veneers, and periodontal treatment.
20% OFF all implant services including abutments.
30% OFF emergency services.
Free Whitening only for adult patients that are cavity-free.

MEMBERSHIP REQUIREMENTS

One Set Fee for head of household or first member
Reduced Fee for each additional member
One Year Membership is for 12 months from your start date
All Fees must be paid in advance.

Treatment Warranty

All warranties will be voided if there are repeated failed appointments
All warranties will become nullified if routine preventative care is not kept up on (2 regular checkups and hygiene visits per year)
All warranties are voided if replacement is due to decay, neglect, or misuse of appliance deemed by Doctor.
